How does this system work?
The patient safety reporting and learning system involves several stages:

- Analysis – specialists evaluate what happened, identifying causes and systemic factors.
- Learning – conclusions are drawn from each case to prevent similar situations in the future.
- Implementing improvements – procedures, communication, equipment, or work organization are enhanced.
